Spinocerebellar Ataxia type\u00a0<\/span>35<\/span>\u00a0(SCA<\/span>35<\/span>) is caused by a genetic mutation that is\u00a0<\/span>passed on<\/span>\u00a0from parents to their children. SCA<\/span>35<\/span>\u00a0leads to problems with balance and coordination, as well as\u00a0<\/span>hand\u00a0<\/span>tremors<\/span>.\u00a0<\/span><\/p>

Spinocerebellar Ataxia type\u00a0<\/span>35<\/span>\u00a0(SCA<\/span>35<\/span>) is a rare neurodegenerative disorder. It is caused by<\/span>\u00a0mutations<\/span>\u00a0in the\u00a0<\/span><\/span>TGM6\u202f<\/span><\/span>gene. SCA<\/span>35<\/span>\u00a0is\u00a0<\/span>very rare<\/span>\u00a0worldwide.\u00a0<\/span>The majority of SCA35 cases have been documented in people of Han Chinese ancestry. However, SCA35 has also been found in people of European and Hispanic ancestry.\u00a0<\/span>The estimated prevalence of SCA35 is less than 1 in\u00a0<\/span>1,000,000 people<\/span>, with fewer than 30 cases reported in\u00a0<\/span>the scientific<\/span>\u00a0literature.\u00a0<\/span><\/span>\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Like many other forms of Ataxia,\u00a0SCA35\u00a0is marked by\u00a0poor balance and coordination. In fact, the word Ataxia means incoordination.\u00a0There can also be problems coordinating muscles that control speech and swallowing.<\/span>\u00a0<\/span><\/p>

Many people with SCA35\u00a0have tremors in their hands. These are intention tremors, where shaking occurs while someone is trying to do a purposeful\u00a0movement with the hand.\u00a0The closer one gets to the intended target of a\u00a0movement,\u00a0the\u00a0tremor becomes worse. For example, if someone with an\u00a0intention\u00a0tremor reaches for a cup, their hand will shake\u00a0more as it gets\u00a0closer\u00a0to the cup.\u00a0<\/span>\u00a0<\/span><\/p>

Less common symptoms of\u00a0SCA35\u00a0include\u00a0cognitive\u00a0impairment and vision symptoms.\u00a0<\/span>\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

SCA35 Prognosis<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

SCA35\u00a0symptoms usually begin between the ages of\u00a013\u00a0and 50.\u00a0The severity of symptoms also varies\u00a0considerably\u00a0between\u00a0people, even within families.\u00a0SCA35\u00a0usually progresses slowly.\u00a0People with SCA35 may need to use a wheelchair\u00a0after\u00a010-15\u00a0years\u00a0of experiencing symptoms.\u00a0Lifespan\u00a0generally is\u00a0not shortened by\u00a0the disease.\u00a0<\/span>\u00a0<\/span><\/p>

Treatments such as\u00a0physiotherapy, occupational therapy, and speech-language therapy can significantly improve the lives of people with\u00a0SCA35.\u00a0Some medications,\u00a0such as Levodopa,\u00a0may be\u00a0helpful for\u00a0symptom\u00a0management.\u00a0Due to\u00a0the\u00a0connection between\u00a0SCA35 and Gluten Ataxia, some clinicians may suggest that people with SCA35 try a strict gluten-free diet as part of symptom management.\u00a0There is mixed evidence to support the effectiveness of a gluten-free diet on SCA35 symptoms,\u00a0mainly due to\u00a0the small number of patients\u00a0available to test its usefulness.\u00a0<\/span>\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

SCA35 Genetics<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

SCA35\u00a0is an inherited genetic disorder. It is caused by an abnormality of a single gene called the\u00a0<\/span>TGM6\u202f<\/span><\/i>gene. The abnormality can be passed from generation to generation by family members who carry it. Males and females are equally likely to inherit the genes that cause\u00a0SCA35.<\/span>\u00a0<\/span><\/p>

Genetic diseases like\u00a0SCA35\u00a0occur when one of the body\u2019s 20,000 genes does not work properly. Genes are microscopic structures within the cells of our bodies that\u00a0contain\u00a0instructions for every\u00a0SCA35\u00a0a person inherits from his or her parents.\u202f\u00a0Two copies of each gene are\u00a0inherited;\u00a0one copy from the mother and one from the father.\u00a0SCA35\u00a0is an autosomal dominant disease,\u00a0meaning that someone will develop symptoms if they inherit at least one mutated copy of the\u00a0<\/span>TGM6\u202f<\/span><\/i>gene. Each\u00a0child of\u00a0someone\u00a0with SCA35\u00a0has a 50% chance of inheriting the gene that causes\u00a0SCA35.\u202f<\/span>\u00a0<\/span><\/p>

Different types\u00a0of mutations in the\u00a0<\/span>TGM6\u202f<\/span><\/i>gene\u00a0can cause SCA35.\u00a0Most of these are point mutations, where a single\u00a0amino acid\u00a0of someone\u2019s DNA is\u00a0changed. Short deletion mutations,\u00a0where\u00a0a few\u00a0amino acids have\u00a0accidentally\u00a0been\u00a0deleted\u00a0from\u00a0someone’s\u00a0DNA, have also been found to cause SCA35.\u00a0While these mutations differ from each other in how they look, they all lead to problems in the\u00a0<\/span>TGM6\u202f<\/span><\/i>gene\u00a0and\u00a0Transglutaminase 6\u00a0protein.\u00a0<\/span>\u00a0<\/span><\/p>

Gene tests can be performed for diagnostic purposes to\u00a0determine\u00a0what kind of Ataxia is within a person or family. Genetic testing\u00a0can also\u00a0be done, in some circumstances, even before there are symptoms,\u00a0to\u00a0determine\u00a0whether a person carries the abnormal gene or genes that cause Ataxia. This is called predictive or presymptomatic testing. A gene test\u00a0can also\u00a0be used to\u00a0determine\u00a0whether a fetus has an abnormal Ataxia gene. This is called prenatal testing. Anyone who is considering a predictive or prenatal test should consult with a genetic counselor to discuss the reasons for the test, the possible outcomes, and how those outcomes might affect the person emotionally, medically, or socially.<\/span>\u202f<\/span>\u202f<\/span>\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

SCA35 Diagnosis<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

A neurologic examination can\u00a0<\/span>determine<\/span>\u00a0whether a person has symptoms typical of\u00a0<\/span>SCA35<\/span>.\u00a0<\/span>A neurologist is often the most helpful specialist in recognizing symptoms and diagnosing the disease that causes Ataxia.<\/span>\u00a0There are several potential follow-up tests. MRI brain imaging is often used to confirm cerebellar atrophy or degeneration.\u00a0<\/span>Clinical exams are performed to measure various movement, behavioral, and speech-related symptoms.<\/span>\u00a0<\/span>However, a specific diagnosis of\u00a0<\/span>SCA35<\/span>\u00a0ataxia can only be made by a genetic test.\u00a0<\/span>This is usually\u00a0<\/span>accomplished<\/span>\u00a0through Whole Exome Sequencing (WES) or Whole Genome Sequencing (WGS). WES and WGS<\/span>\u00a0test results<\/span>\u00a0are complex<\/span>. They<\/span>\u00a0often need to be evaluated by an expert in genetic<\/span>s, such as a Genetic Counselor or Medical Geneticist.\u00a0<\/span><\/span>\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
